MGPI - Modern General Practice Improvement 2024
The Practice is working towards the Modern General Practice Improvement plan this includes updating our telephone communication system to enable us to direct patients to the correct service and give up to date information on the messages. The Practice will be having a new telephone system installed in March/April 2024
The Practice continues to work closely with our Primary Care Network 5 (PCN5) 2025 2026
The Practice and patients are working with 10 other Practices in the area, we are working as one to bring new services needed in this area to our patients.
PCN5 Practices continue to work together to deliver the booster covid vaccination programme, improving flu vaccination uptake and looking at ways to improve services for patients with Learning Difficulties. The health centre has held open days organised by PCN5 for patients with a Learning dissability and flu clinics where patients can drop in
PCN5 are now offering appointments on an evening and at weekends at various locations for patients to attend for their Diabetes, Cervical Smear, Asthma, Bloods reviews so giving patient choice.
ARI CLINICS - ACUTE RESPIRATORY CLINICS
PCN5 work closely with Bradford Care Alliance delivering Enhanced Access Clinics across Bradford for our patients.
FROM 1ST JAN 2025 TO 31ST ARCH 2025 A NEW CLINIC HAS BEEN INTORDUCED FOR PATIENTS SUFFERING SCUTE RESPIRATORY CONDITIONS. ALL PATIENTS PRESENTING WITH THESE ACUT PROBLEMS WILL BE DIRECTED TO THESE CLINICS WHICH ARE RUNNING AT DIFFERENT LOCATIONS AROUND BRADFORD.
CLICS - Ceased 2024
The Clics service has been a successful service for patients with good outcomes and the Practice is sad that it has not been recommissioned by the ICB to continue. We are hoping that another service will be put in place to replace this service.
CLICS is a relatively new service for patients which was introduced in October 2020. Moor Park Medical Practice will be running this service as an individual practice from March 2022. All Practice staff can refer patients into this service where a Community Connector will ring them and help with any social or medical needs. The service has a lead administration person, community connector, ANP Advanced nurse Practitioner and GP input for this service.
Patients referred to CLICS service will be reviewed by a Community Connector and an Advanced Nurse Practitioner so covering social and medical problems. The Community connector has resources to voluntary groups and services and can gain other information for patients whatever that may be. The ANP works together with Community Connector resolving medical issues. All these people come together with the GP's in a MDT meeting regularly
FLU Vaccinations have been offerred to patients from 1st October 2024 to 31st Jan 2025
Any patient requesting the flu vaccination from Feb 2025 will be directed to the pharmacy.
RSV VACCINATION
Patients are being invited to book a vaccination for RSV Respiratory Syncytial Virus which is a respiratory virus common over the winter period. Tel: 01274 778400
PRACTICE NEWS LETTERS
Practice Newsletters are published/printed quarterly a file is on the patient information desk at reception and can be looked at. If any patient would like a copy of the Newsletter then please ask the receptionist.
GDPR
General Data Protection Regulation
The Practice has implementing changes in line with the new GDPR regulation which came into force on 1st May 2018
The Practice has printed posters and information about the new GDPR which are displayed around the surgery. Patients Please note that in order to make appointments, order prescriptions or discuss any other things relating to a patient record Practice staff have to be speaking to the patient or have written permission from the patient to speak to a specifice person. All staff are unable to discuss anything with any other member og the family or friends. We are sorry for any inconvenience this may cause.

NHS GP SURVEY 2023
Over the next few months from January -March 2023 some of our Practice Patients may receive a NHS GP Survey through the post. We would ask that you complete and return the survey as soon as possible. The Survey can also be completed online through the link provided. If patients require any help with the completion of the survey or you have any questions please ask a member of our reception team and they will be happy to assist. Returning the survey is very important for information to be gained about the practice. Thank you for your assistance in this.
Online Services
From April 2016 patients will be able to access there medical records online. The Practice has given full access to Online services for all our patients unless stated not required. If you have lost your password or log in details please ask at reception for your details to be reset and sent out to you again, this can be done by sms text message of we can post them out to patients. Online access enables patients to look at their medical records, book appointments and cancel appointments online, get results, summaries and order prescriptions/medication.
Update 2022 The Practice has given online access to all our patients where this is possible
Accessible Information Standard
The practice has implemented accessible information for patients and aims to make sure that disabled patients have access to information that they can understand and any communication support they may need. this includes making sure that patients can get information in different formats according to their needs.
Health Visitors
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
Care Navigation
Throughout Bradford a new system called Care Navigation is being used. Care navigation is a Service that gets patients to the right service they require. Reception staff and the Practice Manager have all been trained for Care navigation and would appreciate patients cooperation when they ask questions.
