Patients can now register online or visit the practice to register
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photo ID.
The surgery serves patients in the BD2,BD3 and some parts of LS28 area and historic patients living in some parts of BD4

Register with a GP surgery
The Practice follows government guidance when registering patients but also has its own protocols and policies in place to protect patients and the practice. We do ask that patients provide identification, proof of address and their NHS Number where possible. If it is not possible to do this then the process for registering may take a little longer. The practice has previously experienced cases of Identity theft when patients have registered, this caused disruption and delay to the service we offer and caused distress to staff and patients. Managing Identity theft is a difficult and a long process taking many hours of staff time to resolve, therefore in order for this to not happen again we have implemented our policies and protocols for everyone who registers with our practice.

Find your NHS number - NHS
An NHS number is a 10-digit number, like 485 777 3456. Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.
Patients can find their NHS Number on any letter or document you have received from the NHS, including prescriptions, test results, and hospital referral or appointment letters. If you cannot find your NHS Number in these ways, you can ask your previous GP practice to help you
Patients who have no fixed abode or travellers can register at the practice. Please note these registrations may take a little longer to process bur be assured these applications will be dealt with in a timely manner so patients can access GP appointments appropriately.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Please note that all patients are required to attend the New patient Health Check Appointment. and any patients who fail to attend the New Patient Health Check will not be registered. The Practice has a policy on patients not attending pre booked appointments and patients can be removed from the Practice for DNA’s
New Born Babies- To register a new born baby, it is essential that you provide the practice with the NHS number (this can be found in the front of your babies red book) along with a copy of the birth certificate. we ask that New Mums bring the Red Book to the Practice when attending the first Postnatal/Baby Immunisation appointment
