Appointments
UPDATED March 2024
The Practice continues to offer face to face and telephone appointments giving patients choice. We ask that patients can give as much information as possible to the receptionist in order for them to Care Navigate and direct you to the correct service.
The Practice Offers the following ways to book appointment's
Appointments can be booked on the day by telephoning the practice telephone lines open from 8.00am
Appointments can be booked in advance by asking at reception, these can be telephone or face to face appointment's
Appointments can be pre booked by patients by telephoning the practice or going online. Patients who pre book have a choice of GP/Clinician and the day and time
Appointments can be pre booked online for face to face or telephone appointment's
Appointments can be booked and cancelled Online through our Websites System online Services
Appointments can be booked on the day with Enhanced services where patients will be seen at another location
Appointments can be pre booked for telephone or face to face appointments with Enhanced Access at a time and day that suits the pateint including evenings and weekends
Appointments can be booked at the Practice with a Nurse or HCA these can be face to face or telephone if appropriate
Appointments can be booked at Enhacned access where they will be seen on an eveing or weekend with a Nurse giving patients choice
The telephone lines are open at 08:00am and we ask that patients telephone as early as possible to book on the day appointments.
On the day appointments will be with any of the Clincians available. if you do wish to see a particular GP/Clinician we ask that book in advance by telephone in person at reception or by using the Online Services.
Enhanced Access
This service enables patient to be seen at other locations. These appointments include GP, ANP, Nurse, HCA, Mental Health, Physio and Welfare Benefits help. These appointments are on evenings and weekends and can be booked by telephoning or calling in at our reception. Appointments can be booked on the day by telephoning after 9.00am and can also be booked in advance with Male or Female clinicians.
The Practice offers appointments with Doctors, Advanced Nurse Practitioners, Trainee Advanced Nurse Practitioners, Pharmacy Practitioners, Nurse and Health Care Assistants and Social Prescibers
Enhanced Access Appointments telephone or face to face
This service enables patient to be seen at other locations. These appointments include GP, ANP, Nurse, HCA, Mental Health, Physio, Bereavement counselling, Welfare Benefits help and many more. These appointments are on evenings and weekends and can be booked by telephoning or calling in at our reception. Appointments can be booked on the day or can be pre booked for patient choice so patients can see a Male or Female clinician. The service offers telephone or face to face appointments.
PCN5 - Appointments can be booked on an evening and saturday morning. These appointments are for all patients within the PCN5 area covering 10 Practices. Appointments can be pre booked at a time and date that is suitable for the pateint. The clinics cover the following, Diabetes, Asthma, Bloods/BP, and Cervical Smear
The first point of call with an ailment for patients should be the local pharmacy where they can assess you and provide you with information and medication if needed. Pharmacist are able to do a consultation for minor medical conditions.
On the 31st January 2024 the government launched a new medical service called Pharmacy First where patients can have a consultation with a pharmacist of their choice anywhere in the country. Over 95% of Pharmacists across the country have signed up to the new government scheme and will offer the service. Patients can now book consultations with a Pharmacist for the following acute medical problems:
Acute Otitis Media* Ear and throat, viral
Impetigo
Infected insect bites
Shingles
Sinusitis
Sore throat
Uncomplicated urinary tract infections
Women 16-64 years
Appointments can also be booked at the pharmacist for the following:
Blood Pressure Reviews and Contraception Pill
Staff at Moor Park Medical Practice will care navigate all patient enquiries and calls so patients are directed to the correct service for their ailment or problem they have.
The Practice is working with local Pharmacies and Bradford Clinical Commissing Board to bring this new service to our area. Patients will ring the Practice for appointments as normal but will then be triage and if the problem is for minor ailments then they will be given an appointment with a local Pharmacist to look at the problem.
Urgent Appointments
Urgent appointments are available at the Practice, patients are asked to contact the reception team and give information as to why the Appointment is being requested as Urgent this will be acted on accordingley
Emergency Appointments
Patients who have an emergency need to telephone the surgery as soon as possible and the doctor will assess the matter. Reception staff may have to take a few details to pass to the Doctor. If the doctor asks a patient to come to the surgery regarding an emergency then the patient needs to be available to come down straight away. However due to Emergencies being seen in between other patient appointments patients may have to sit and wait to be seen. Please note emergency appointments may not always be with the doctor of your choice.
If a patient is ringing from a telephone box then please inform the receptionist who will ring you back
Chaperone Service
If you would like a chaperone present during an examination please let us know when making your appointment. notices are displayed around the Practice asking if you would like a Chaperone.
E Consult & PATCH's
The Practice offers E CONSULT and PATCHS which means Patients can now go to our website and fill a form in to submit to the practice, this form will be sent to the GP and actioned accordingly. E Consults can be made for medical reasons or administration requests. This is an easy way to communicate with the Practice and your E consult will be directed to the right person to deal with. If it's a medical problem the practice will get back to you within 48 hours and if it is a administration request this can take up to 72 hours
Patients requesting repeat sick notes are directed to this service
During 2022/23 the practice noticed an increase in patients using this service and have had good feed back.
Patients can use E Consult on our website to contact the Practice for Medical and administrative queries. A Blue box is on our home page and patients just click the box to be taken through the E Consult

Specialist and Hospital Care
If a GP or another member of our healthcare team believes you need hospital treatment or specialist care elsewhere, they will advise you at the time of your consultation. The GP will offer a choice of consultant and hospital. A referral will then be made to the hospital as soon as is deemed necessary.
All appointments and consultations in specialist care will be copied in writing to the practice and your progress will be monitored and recorded in your notes.
Cancellations
If patients are unable to attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else. The Practice ask that patients give 24 hours notice for a cancellation. Failure to cancel an appointment on 3 occasions may result in removal from the practice list.
